Before filling in this form please contact Learning Technology Services at lts@yorku.ca to ensure your requested time & date is available.

Please note that this is for a VIRTUAL session and support ONLY. If you require support for a hybrid or live session or event, please reach out to us at the aforementioned contact address.

Once you have successfully submitted the webinar request form, on-screen confirmation text will be displayed, and you will be sent a confirmation email. This is not final confirmation that your webinar has been booked, only that the request form has been submitted. Once your webinar has been created LTS will send an email with confirmation, and link(s).

This form allows the option to save your progress and come back later to finish.

Please note that once a form has been submitted and the webinar has been scheduled, any cancellation is subject to an $85 administration fee.
